MESSAGE 

MEANING / ACTION 

DATE out of seq 

You have entered a Start Date on or before the start date 

of the most recent relationship already recorded for this 

Practice. (If the previous relationship is one that was 

 merged  into another Practice, you will also get this 

message if the Start Date entered is not later than the 

End Date of the previous relationship.) 

Enter the correct 

Start Date.

P'SHIP invalid 

If a partnership code is entered, it must be in the format: 

`P/', followed by the partnership code; e.g. `P/000020'. 

Enter a valid partnership code.

P'SHIP/GP conflict 

If a partnership code is entered, you cannot also enter 

GP codes. 

Remove either the partnership code or the 

GP code(s).

P'SHIP unknown 

You have specified a partnership code which does not 

exist on your HA database. 

Enter either a partnership 

code or GP codes which are known to your HA.

GP/Start conflict 

The entered Start Date must be later than any on record 

for the GPs entered. It must also be later than any `end 

date' which may exist for the GPs due to them having 

been `merged out' at some time, or due to them having 

been within the last relationship of a Practice which has 

been closed. 

Enter the correct Start Date.

GP missing 

You have not specified either a GP code or a partnership 

code.  

Enter either a partnership code or at least one GP code.

GP duplicated 

The same GP code has been entered more than once. 

Enter the GP code once only.

GP unknown 

You have specified a GP code which does not exist on 

your HA database. 

Enter either a partnership code or 

GP codes which are known to your HA.

GP in use 

You have specified the code of a GP who is currently 

specified in another Practice.  

Either correct the GP code, omit the GP from this 

Practice, or remove the GP from the other Practice.

GP not allowed 

If a GP code starts with  ZZZ , the last 3 characters of 

the code must not be < 100. 

GP not started 

You have entered the code of a GP and either: 

the GP's Start Date is after the relationship Start 

Date, or 

the Start Date on an existing relationship has been 

changed and one or more GPs within the
